We recommend that you familiarize yourself with your insurance policy’s requirements. Please direct your questions regarding your insurance policy coverage to your insurance company. There is a phone number on the back of your insurance card.
Self-Pay
Payment for self-pay patients is due at the time of service. We accept most major credit cards, personal checks and cash.
Copays
Copays specified by your plan are collected when you arrive for your visit. Your insurance company requires that you be rescheduled for another appointment if that is not possible. To avoid missing your appointment, please bring some form of payment, credit card, check or cash.
Insurance Coverage – Are you covered if you see us?
Prior to scheduling your appointment with our office, we highly recommend that you contact your insurance company to make sure your visit will be covered. Plan restrictions can include geographic limitations, or may limit you to a specific authorized provider.
Medicare covers the hearing exam if your physician orders it for medical necessity. We must have an Rx (order) on file to submit your claim for processing.
Medicaid: We no longer accept Medicaid in our office.
Affordable Care Plan: It is highly recommended that you check your plan for hearing coverage.
Contracted Insurance
Broadwater Hearing Care, Inc. participates with most major insurance companies. Contracted insurance companies will be billed for charges incurred during your visit.
Non-Contracted Insurance
Should you carry a non-contracted insurance policy, you will be responsible for paying any charges in full at the time of your visit. We will bill your insurance one time, as a courtesy to you. You will receive any monies reimbursed by insurance. Please note that you are responsible for the full balance.
Referral Q&A
Your insurance may require that you have a referral prior to visiting a specialist. If you are not sure, please contact your insurance provider. Failure to follow your policy’s rules will result in additional out of pocket expenses. You are responsible for obtaining referrals and will be billed the full balance should you fail to obtain and provide the necessary referrals.
Contact your primary care physician office
Obtain a referral from your PCP to see a specialist in your network
If the specialist refers you to another specialist, you must first obtain another referral from your PCP for the new specialist.
Statements
We have a 30 day billing cycle. Any balances are due 30 days from receipt.
